Accessible transportation

 

Dear Valued Client,

It is with deep regret that we must inform you that the Sudbury-East Accessible Transportation program will no longer be available after March 31st, 2025. While this is not the outcome we had hoped for, Sudbury-East Senior Support remains committed to finding an alternate solution to provide accessible and timely access to medical appointments for residents residing in Sudbury-East. In the meantime, transportation for medical appointments through the volunteer transportation service currently offered by Sudbury-East Senior Support may be available for residents over the age of 65. Thank you for your support over these past few years. It has been a great pleasure to assist you with your transportation needs.

SEAT program - This program is a support service that provides non-emergency transportation to medical appointments. Transportation is provided to individuals who meet the eligibility criteria by paid drivers using a wheelchair accessible van.

The service is available Monday to Friday. If an on-going medical treatment or a medical apt can only be booked after hrs, arrangements may be made to accommodate the appointment if the driver and resources are available.

This service is available to residents residing in the Municipalities of French River, St Charles, and Markstay/Warren.

The Municipalities of French River, St.-Charles and Markstay-Warren, alongside the Warren and District Lions Club and Univi Health Centre have proudly partnered to offer its residents a transportation service to medical appointments. This service, called The Sudbury-East Accessible Transportation Program (SEAT), provides accessible, equitable and timely access to health related appointments to residents residing in Sudbury-East.

IMPORTANT NOTES:

The vehicle is accessible.

The service is available Monday to Friday and rides are scheduled according to the appointment time, location and driver availability. If an on-going medical treatment or medical appointment can only be booked after hours, arrangements may be made to accommodate the appointment if the driver and resources are available.

Clients are asked to schedule 5 business days prior to the appointment day. All rides must be booked during office hours Monday to Friday, from 9:00 am – 4:30 pm. Cancellations must be made within 48 hours of the trip.

The SEAT program is a 5 year rural transportation program financed through the Community Transportation Grant Program.


Meals on Wheels

 

A support service which provides a frozen meal to an individual to meet his/her nutritional needs. The meals are delivered by volunteers. This provides a regular social contact and check-up of the client.

Assisted Living

 

Supportive housing and aging at home programs are designed to provide personal support and homemaking to the elderly who require services on a routine basis and have access to a 24 hour on-call emergency response service. Arrangements are coordinated by the staff of Sudbury-East Seniors Support.

In all of our programs, both family and client are involved in the decision-making process of the agency’s plan of service. We work in collaboration with all other service providers so the client will get as many services required as possible.

Emergency Response

 

Philips Lifeline
A service which provides an electronic device in a client’s home so that the client can communicate, in an emergency, with a center staffed 24 hours a day that can summon emergency help.


Respite

 

A support service provided in a client’s home to enable family or friends to have relief from their care giving role and to help relive stress placed on family relationships.


Homemaking

 

A support service provided to assist with routine household activities such as light housekeeping, laundry, ironing, planning menus, some meal preparation, and other services as the need exists.
